Consultation Care Rate for Established Partners

Acute Care- Only Available During Office Hours at this time: MOST Tuesdays, Wednesdays, and Thursdays from 9AM-2PM

  • $25 per 15 minute virtual or telehealth visit during office hours only. 
  • If you need a longer appointment, plan on the above hourly rate per 15 minute increments. This fee will also be applied to management time- for example: lab requests, lab orders, lab review, referrals.
  • You decide what you pay, based on how much time you need.

  • Examples of an acute care issues my scope of practice covers are as follows:  ear-ache, sinusitis, or poison ivy dermatitis.
  • If you have an emergency situation, please go directly to the hospital emergency room or urgent care facility for evaluation and treatment.
  • When you need to schedule an appointment, please utilize your Patient Fusion Portal OR you may text me. Please do not text specific details. Simply state that you would like to schedule an “acute care” or “primary care” visit. I will respond to you during business hours and we will schedule your appointment.

 

Chronic Care- Only Available During Office Hours at this time: MOST Tuesdays, Wednesdays, and Thursdays from 9AM-2PM

  • $25 per 15 minute virtual or telehealth visit during office hours for chronic care consultation with basic follow up.
  • You will be billed for ongoing care for a chronic health concern.
  • Examples of chronic care consultation could include treating high cholesterol, blood pressure, or migraine headaches.
  • Additionally, you will be billed for management time. Examples: discussing labs and management options, ordering labs, reviewing labs, referrals, and refills.
  • If you are requesting a refill and I have not spoken with you in more than 3 months, please schedule a visit with me regarding that refill request.

An example of billing for a chronic care health concern could look like this:

  • First Visit:  Billed based on time spent. I am not currently taking any new patients.
  • Second Visit:   A few months later you need a refill on your prescription or you feel you need to change what you are doing for treatment. This visit would be billed as a separate consultation fee for a chronic health concern.
  • Third Visit:  Recheck labs and adjust medications. This visit would be billed as a separate consultation fee for a chronic health concern.
  • If you have an emergency situation at night, please go directly to the hospital emergency room or urgent care facility for evaluation and treatment.
  • When you need to schedule an appointment, please utilize your Patient Fusion Portal OR you may text me. Please do not text specific details. Simply state that you would like to schedule an “acute care” or “primary care” visit. I will respond to you during business hours and we will schedule your appointment.

General Costs and Considerations

I do not prescribe any controlled substances like opioids for pain, or ADHD therapeutics. I also do not prescribe hormonal contraceptives. If you are in need of these services, I will refer you to providers who prescribe these types of medications.

* You will be responsible for paying your lab, radiology, or pharmacy directly for services rendered. Office hours will be available through the patient portal. I will respond to texts and offer consultations during this time. I am not available 24/7. Should you have an emergent need outside of office hours, please seek care at Urgent Care or the Emergency Room.

Payment For Services

Cura Personalis Wellness is a cash for service practice, which means I do not accept any form of insurance. Payments can be made via Zelle, cash or check only.  Payment will be expected the day service is rendered, that being said, if you find yourself in financial hardship, please talk to me. Please wait to pay until I send an invoice.

*Any unpaid bills exceeding 30 calendar days, without discussion of hardship, will result in your patient status being deactivated.  This means that you will no longer be a patient of CuraPersonalisWellness and I will no longer be able to provide you care.    I will notify you and refer you to the provider of your choice in the event this occurs.
