Due to the in-depth nature of this work, I take a very limited number of cases each year- approximately one per month. If you are interested in being one of those cases or being added to the wait-list, please reach out via the contact page.

 

  • I will send you a “Patient Consent to Care” contract, intake documents, and general information regarding the provider-patient relationship at Cura Personalis Wellness for review.  It is my hope this information will guide your expectations of our partnership.  If you choose pursue care, please return those documents to me for review.  I only accept patients I believe I can help.  If your case is beyond my scope of practice, I will refer you to a more appropriate provider.  If I believe our partnership will benefit your overall wellness, I will proceed in building your Electronic Medical Record (EMR).  I will also send you an invitation to your Patient Fusion Portal, which enables secure communication regarding your healthcare needs.  Please activate this account promptly, as it will be our primary means of communication during your care.
  • We will utilize Zoom.us for virtual visits. This platform is Health Insurance Privacy and Accountability Act (HIPAA) compliant and secure, which protects your personal information.
  • Prior to our initial consultation, I will send you a Zoom link via your Patient Fusion Portal so that you will be able check in and participate in our virtual consultation.
  • Once you are an established partner, I will send you a secure link via your Patient Fusion Portal that will grant you access to Cura’s weekly schedule. That link will enable you to schedule and cancel appointments at your leisure. ONLY active patients have access to Cura’s schedule.
  • Instructions regarding acquisition and cost of labs will be sent to you once we have established your Electronic Medical Record and Patient Portal Access.
  • Your Patient Fusion Portal will give you access to your care plans.  Please sign in and review your care plan after each appointment, as I will highlight our objectives for each visit in this area.  Your invoice for care will also be found at the bottom of each care plan.  Due to Cura being a Direct Primary Care practice, payments will be made directly to Cura Personalis Wellness.  I DO NOT work with ANY insurers, as Cura is a fee-for-service practice.  Please remit payment within 30 days of invoice.  If payment is not remitted within 30 days of invoice, you will no longer be considered an “active” patient at Cura and the patient-provider relationship will be terminated.  If you are in hardship, please speak directly with me about your situation.  I’ve been in hardship, and understand that sometimes life hands us more than we can handle.  Please, talk with me if this is the case.
